Describing user selection and population screening strategies in the case management program.
Analysing the comprehensive and integrated evaluation system based on collaboration between professionals.
Establishing objectives and plan interventions.
Determining the implementation of coordination-based interventions.
Analysing the monitoring system during the case management programme: monitoring and evaluation of health and agreed actions.
Designing a planned discharge from the case management program.
