Advanced healthcare training designed to develop the skills of doctors, nurses and social health workers to respond to the new needs of chronic patients, providing care centred on the person and their affective environment, comprehensively attending to their needs, wishes and preferences, and participating in their empowerment in making decisions about their illness.

Objectives
- Know how to address chronic patients. in complex acute or subacute situations at any level of medical intervention: hospital care, primary care, social healthcare or residential care.
- Gain an understanding of the tools that help to interconnect all healthcare devices to achieve effective, efficient actions.
- Know how to extend patient care to their emotional environment and carers.

- Mainly nurses, doctors and social workers.- In addition, the programme is open to other professional profiles with a university degree in the field of health who are professionally related to the Chronicity sector, subject to admission requirements.
